New Requirements for 2016 NYS Tax Return

If you have not yet sent us your 2016 tax information, please fill out this form for you and your spouse. When completed you may upload via our secure online portal or mail to our office at:

In an effort to help protect your identity, New York State requires additional identification to be provided to e-file this year for both the taxpayer and his or her spouse.

Acceptable identification includes:

  • driver’s license or
  • state-issued ID number,

And must include:

  • ID number,
  • document number (see below),
  • issue date,
  • expiration date, and
  • state of issue.

 

Omitting any of this information may delay return and refund processing.  Please provide this information accordingly in your tax organizer or contact your Lumsden McCormick tax provider to send via our secure portal.

Most other states do not require this information to file your taxes but it could help expedite how quickly your return is processed. It will not affect your other state return's submission.
